The Department of the Navy's Freedom of Information Act (FOIA) Program is decentralized to provide you with faster response times to your FOIA requests. We have several FOIA requester service centers to assist you in processing your requests.

FOR MILITARY PERSONNEL RECORDS (which includes the DD214): Please read this carefully.

Active duty Navy and reserve members (including Individual Ready Reserve (IRR)) and for records of members that were retired, discharged, or died while in service since 1995:

The letter should contain first, middle, and last name and the last four of the social security number (and/or enlisted service number/officer file number), rank/rate, designator, military status, address, and signature of the requester.
 

 Secondary Locations: Personnel Offices and Personnel Support Detachments providing administrative support for the local activity where the individual is assigned. Official mailing addresses are published in the Standard Navy Distribution List (SNDL).
